CR DOC will handle your regular checkups, provide care when you are sick, help coordinate your specialty medical care, and look after your overall health.

At CR DOC we also have extended hours in the event a sudden illness occurs. Our office offers urgent care appointments during the week as well as the weekends.

Read over the information in the packet and fill out the forms. After you fill them out, bring the completed forms for your first appointment.
