CR DOC INSURANCE POLICY
In order to find out if CR DOC is a provider for your health care plan there are many options available to you. The first is to look on the back of your insurance card for a toll-free number. Call it to find out if CR DOC, Dr. Roger Cagle, M.D. is a provider participating in your plan.
Another method is to do a provider search on your insurance plan's website. Again, most of these are listed on the back of your insurance card. If you don't know or you do not have ready access to your provider's web address you can easily find it using an internet search engines such as Google or Yahoo.
Finally, our Insurance Department will be happy to assist if you are having difficulty. We will be glad to help you in your research or will find out for you if we are provided with the necessary data.

- Specific name of your plan. Example: Blue Cross, ARKids, Medicare, etc.
- ID Number and Group Number
- Name of the Primary Card Holder
- Date of Birth of the Primary Card Holder
- Name of the Patient being Treated
- Date of Birth of the Patient being Treated
We currently participate in around two hundred health care plans including but not limited to Blue Cross, Medicare, Medicaid, Missouri Medicaid, Tricare, and ARKids. Our participation in any particular plan is subject to change at any time, so it is not practical to list all of them.
CR DOC accepts and files insurance as a courtesy to our patients. However, should a patient's plan not pay for any reason, the patient or their guarantor is ultimately responsible for payment.
IF YOU HAVE NO INSURANCE
If you have No Insurance,
Payment is expected IN FULL at the time of your visit. Cash, Checks, Debit Cards, and All Major Credit Cards are accepted. However, no Temp Checks will be accepted for services rendered.
A list of fees is listed below:
- First Time Office Visit: $55.00 to $165.00
The price depends upon:
- The amount of time spent with the patient
- The severity of the problem
- The different problems being addressed
- Current Patient Office Visit: $35.00 to $95.00
The price is determined by:
- The amount of time spent with the patient
- The severity of the problem
- The different problems being addressed
- Flu Shots: $22.00
- Department of Transportation (DOT) Physicals: $78.00
